Requesting external services
Lavington Public School provides access to external providers. Providers do not have an automatic right of access to a school. Any request for access to a student by an external provider funded by the NDIS (or other scheme that provides funding to individuals) must come from the student’s parent or carer in writing.
Parents must collect a request form from our school office in the first instance. All communication regarding these appointments will be handled between the school and the family.
Please note, we have limited space and availability for school-based appointments. All appointment requests will be reviewed on a case-by-case basis from Term 1, Week 5. All approved appointments will commence from Term 2 - Term 4 only.
To meet the requirements of the school curriculum, and in the best interest of the student and the school, external providers will be able to attend Lavington Public School after 11.20am, for up to 40mins, Monday to Friday on school days. It remains the parent's responsibility to advise providers if their child will be absent from school or if there is a school activity on the day of support.
To engage external therapist support at Lavington Public School:
Step 1:
Parents please fill in a Parent/Carer Request Form and return to the office.
If the school is in agreement with the request, progress to step 2.
Step 2:
Parents contact the therapist, and the service provider completes the requirements and documentation.
Therapists must read:
- Code of Conduct policy
- Controversial issues in schools policies and procedures
- Allegations against Employees in the area of child protection
Therapists must complete and provide:
- Working with Children Check (WWCC) clearance status via a Digital Declaration (not applicable if providers if part of the Specialist Allied Health and Behaviour Support Provider Scheme)
- Completion of Current child protection training for provider staff in the current year (not applicable if providers if part of the Specialist Allied Health and Behaviour Support Provider Scheme)
- Evidence of insurance policies in the name of the organisation (sole traders only need to provide professional indemnity and public liability. Workers compensation insurance must be provided for all other organisations, not applicable if providers if part of the Specialist Allied Health and Behaviour Support Provider Scheme)
- Complete the Parent/Carer request form with the parent, detailing the service delivery schedule and facilities/equipment to be used during the delivery
- Enter into an Engagement Agreement with each school they visit - this agreement will be provided to the therapist upon request
- Provide Engagement Agreement/Attachment B/Form 1: Consent Form to the parent to sign prior to services commencing, and give a copy of this to Lavington Public School - this form will be provided to the therapist upon request
- Provide Engagement Agreement/Attachment B/Form 2: Therapy Update Form after each session to Lavington Public School - this form will be provided to the therapist upon request
Please note, individual therapists are not to sign the Engagement Agreement on behalf of the provider organisation, unless they have authority to bind the organisation. The Agreement must be signed by two directors, or officers from the provider organisation with authority to bind the organisation to the Agreement.
Subcontracting arrangements are not permitted for services delivered by externally funded service providers.
Parents return all documentation to the school.
Step 3:
Lavington Public School will send an email to the therapist, teacher and parent. After a suitable time has been negotiated the therapy can commence.
Therapy is not to commence until therapists have received an email or phone call approving the request.
This process is to be completed at the beginning of each school year.
